Notice of Privacy Practices
Western Health Screening (WHS), a non-profit organization, is committed to protecting the confidentiality of any and all participant laboratory test results and all other personal health information (PHI) collected or created in connection with all participant blood screenings purchased or initiated through Western Health Screening.
Participant demographic information is acquired and disclosed in compliance with the HIPAA Privacy Rule and Security Rule listed below:
The HIPAA Security Rule establishes national standards to protect individuals’ electronic personal health information that is created, received, used, or maintained by a covered entity. The Security Rule requires appropriate administrative, physical and technical safeguards to ensure the confidentiality, integrity, and security of electronic protected health information.
The HIPAA Privacy Rule establishes national standards to protect individuals’ medical records and other personal health information and applies to health plans, health care clearinghouses, and those health care providers that conduct certain health care transactions electronically. The Rule requires appropriate safeguards to protect the privacy of personal health information, and sets limits and conditions on the uses and disclosures that may be made of such information without patient authorization. The Rule also gives patients rights’ over their health information, including rights to examine and obtain a copy of their health records, and to request corrections.
The Security Rule is located at 45 CFR
Part 160 and Subparts A and C of Part 164.
Participant information is acquired:
• Directly from the Participant verbally at the wellness event
• from WHS’s computer database, collected in previous year(s).
• WHS’ online registration program
Demographic information is transmitted to Quest Diagnostics Laboratory Inc., a clinical laboratory in Denver, CO, and used solely to process and identify your specific blood sample and the screenings you have requested. A report booklet is printed with your demographic information and screening result(s). The report is mailed directly to you at the address you have provided WHS at time of service.
Access to your PHI is restricted to authorized personnel and/or volunteers on a need to know basis, thus enabling us to provide service(s) to you. We maintain a database with physical, electronic and procedural safeguards that comply with federal standards to guard your personal health information.
For your protection your BLOOD SCREENING RESULTS will be mailed to you at the address you have provided WHS. To request your blood screening results be sent to a third party, or you need to change the address formerly given and have your results mailed there, a Consent & Release form must be submitted to Western Health Screening by you, signed, and dated.
WHS does not market or sell participants demographic information.
